Not a bad picture, but difficult puzzle. I found myself resorting to guesswork until near the end. Could there be some translation problem in the title? Vacation instead of vaccination perhaps??

No, I googled it and it is vaccinations. The hippo is scared so he doesn't get them but then he gets sick and miserable because he didn't get his vaccinations. Moral of the story is get your vaccinations. Soviet propaganda I can agree with.
